Welcome To Shotfield Medical Practice

Self Care Week – 13-19 November 2017

Self Care Week is an annual national awareness week that focuses on embedding support for self care across communities, families and generations.

See below for various useful links and information around self care

Please note that we are now making appointments for adults aged over 65 and those in at risk groups and also children aged 2-3yrs (pre-school only) and those children in an at risk group.  Please note all children who are in reception to school year 4 will be offered nasal flu at school, we will not have sufficient supply to offer them at the surgery so please do not request it to be done here.

Appointments are in Saturday morning clinics on 23rd September (adults only), 7th October, 21st October and 4th November.   Patients with a long term condition who are due to have their annual review in the period Sept-Dec 2017 will be given their flu vaccination at that appointment to save them attending twice.  Please ring our reception team on 020 8669 7612 option 1 or call in to make your appointment.

The Role Of A General Practice Nurse

Health Education England have produced a video to help promote the role of a General Practice Nurse.  General practice nursing is an interesting, varied and rewarding role.  Please see the video here:

Please do not ring the practice about your test results, you can apply for online access if you do not already have it, and see all your results there and print copies if you wish, we will no longer be providing hard copies from the practice. If your test result is abnormal you will always be notified. Thank you for your assistance.

Please note that we are currently unable to order Hep A and B vaccines so this may affect your ability to get all your travel vaccinations done at the practice if you need to do so. Alternatively you can contact a travel clinic to see if they can provide these to you.

We will update when the supply is made available to us again.

Extended Access Services Hub

Patients registered in Sutton can now access appointments with a GP or a nurse (for certain items) outside normal opening hours in the evenings or at the weekends.

There are now two hub sites in Sutton.  One at Wrythe Green Surgery, Carshalton.  This is the surgery situated on Wrythe Lane and should not be confused with Green Wrythe Surgery and the other at the Old Court House Surgery in central Sutton.  You can only attend the hub by appointment it is not a walk-in facility.

Please see here for full details on how to make appointments via your own surgery (you cannot contact the hub directly) and where to go when you have made an appointment.

Please note if you are given an appointment in the hub and you need to cancel it you need to telephone 07578 813838.

Sick Certificates

Please request these on the day they are due to be re-issued only, they cannot be issued in advance.  They will not be ready for collection until after 4pm on the working day after they are requested at the earliest as we need time to process and get them signed.  Please note you can also request these now via the online form, click here to access the online form.

Our Doctors are:

Drs Lings, Lewis, Munden, Mahfooth, Pattani, Kansagra, Fisk, Martinez & Rodriguez-Benito.  Dr Fisk is currently on maternity leave.

We are one of two GP practices located at this site. We are not responsible for all other services at this site which includes community and hospital services. We are a friendly team whose aim is to look after you and the health needs of your family.
