Please help us to keep our records up to date by letting us know if you change your name, address, contact telephone numbers or email address. If you move house please check whether your new home is still within our practice area. If it is not then you need to advise us of the new address and you will be given 28 days to locate a new GP in your area. You will need to use the relevant form available at reception to notify us of any change of name, address or other contact details.
Please note that we may use your email address or your mobile phone number to contact/text you about reviews, appointments or other general information. Providing these details and signing the registration form indicates your consent for us to do this. Please ensure your email address and mobile phone number is always kept up to date.
