Register With The Practice?
Obtain relevant forms and paperwork from our reception, this is open 8am to 7pm Monday to Friday. When completed forms are submitted you should allow 7-10 days for the registration process to be completed. All forms must be completed clearly and in full otherwise they may be returned to you for further attention. You will need to provide information about your previous GP, including their name and address, and your own NHS Number. If you do not have this you will need to obtain it from your previous practice.
For all adults registering, we also require 1 original item of ID in your own name & giving the full address at which you are registering to be submitted with all other registration paperwork. This item of ID should be dated in the last 3 months (ie bank statement or utility bill), items such as a driving licence or passport do not qualify. Do not submit any paperwork until all items are entirely complete & the ID is also available. If you have recently moved to a new address and do not have a recent ID please wait to register when this is available.
All patients registering who are over 16 yrs of age will be asked to see the Health Care Assistant for a brief new patient medical. The check allows us to familiarise ourselves with your past medical history and any current medical problems you may have. The check also allows you to ask any questions you may have about the practice and the services that we offer. You will appreciate that it can take a few weeks for us to receive your records from your previous doctor, and it is important that we know of any significant history or problem that you may be suffering from.
You would normally be unable to see a GP or nurse here until your registration is complete.
If you are going to require repeat medication soon after registration without seeing a doctor please obtain full details from your previous GP & pass it to the receptionist with your written repeat prescription request. We will be unable to issue repeat precriptions without an appointment with a doctor if this information is not provided.
Please ensure your personal information is always kept up to date and inform us immediately of any changes.
Once registered, we have an online service for booking and cancelling routine appointments with the doctors & repeat ordering of standard items of medication (not changes), please ask at reception for more information on how to register for this service or CLICK HERE to download.
Summary Care Record
If you DO NOT wish to have your basic health information uploaded to the national summary care record (SCR) please complete the relevant form & return it to us with your other paperwork. The basic information includes medications, allergies and adverse reactions only, no other data would be included in the upload. If you do not object to this upload you do not have to complete the form, the upload will then take place automatically at some point.
NHS Organ Donor Register
Whilst there is a section to join the NHS Organ Donor Register on the GMS1 form you complete on registration with the practice please note that currently if you enter your details there that will NOT result in you being added to the register. This is because previously this was undertaken by the Primary Care Support Services and due to reductions in staff they are now unable to fulfil this function and have asked practices to undertake this work but unfortunately we do not have the adequate admin staff resource to do this either. There are also delays to us being able to complete this without the PCSS first accepting registration with the practice and having full NHS number at registration.
In view of this you should note that if you do wish to join the NHS organ donor register you should please go to www.organdonation.nhs.uk and do so online telephone 0300 123 2323. We apologise for any inconveniece caused.
How Do I....
Change My Details?
Please help us to keep our records up to date by letting us know if you change your name, address, contact telephone numbers or email address. If you move house please check whether your new home is still within our practice area. If it is not then you need to advise us of the new address and you will be given 28 days to locate a new GP in your area. You will need to use the relevant form available at reception to notify us of any change of name, address or other contact details.
Please note that we may use your email address or your mobile phone number to contact/text you about reviews, appointments or other general information, providing these details & signing the registration form acts as a consent for us to do this. Please ensure your email & mobile phone number is always kept up-tp-date.
