Register With The Practice?
All patients registering with the practice are asked to see the Health Care Assistant for a brief new patient medical. The check allows us to familiarise ourselves with your past medical history and any current medical problems you may have. The check also allows you to ask any questions you may have about the practice and the services that we offer.
You will appreciate that it can take a few weeks for us to receive your records from your previous doctor, and it is important that we know of any significant history or problem that you may be suffering from.
All forms must be completed in full and clearly otherwise they may be returned
to you for further attention.
NHS Organ Donor Register
Whilst there is a section to join the NHS Organ Donor Register on the GMS1 form you complete on registration with the practice please note that currently if you enter your details there that will NOT result in you being added to the register. This is because previously this was undertaken by the Primary Care Support Services and due to reductions in staff they are now unable to fulfil this function and have asked practices to undertake this work but unfortunately we do not have the adequate admin staff resource to do this either. There are also delays to us being able to complete this without the PCSS first accepting registration with the practice and having full NHS number at registration.
In view of this you should note that if you do wish to join the NHS organ donor register you should please go to www.organdonation.nhs.uk and do so online telephone 0300 123 2323. We apologise for any inconveniece caused.
How Do I....
Change My Details?
Please help us to keep our records up to date by letting us know if you change your name, address or both day and evening telephone number. If you move house please check whether your new home is still within our practice area.
