Important Information for PatientsPractice Move to New PremisesThis is to advise you that the practice will be moving to the newly developed building which will be known as The Jubilee Health Centre. The full address for our practice will be: Shotfield Medical Practice The moving date is currently expected to be Friday 18th May-Monday 21st May. There is a booklet giving more information to patients and visitors about the new Health Centre so if you have not already seen one please ask for one at reception. In particular, please note that patients will be asked to refrain from attending or phoning the surgery on Friday 18th and Monday 21st May to allow the move process to run as smoothly and quickly as possible. We will of course appropriately provide for any emergency medical requirements during this period using our own doctors or the out of hours service Patient Care 24. If you are likely to need to be seen again around this time please make any non-urgent routine appointment for a date before or after our proposed move. If you are likely to need a repeat prescription around this time please ask for it the week before so we can get it processed and to you before the actual move date and to remove the need for you to call in to collect. We really appreciate your assistance and co-operation in this regard.
