To manage demand and prioritise vulnerable patients, please submit only one medical request per form, as we cannot handle multiple requests in a single submission.
Please familiarise yourself with Pharmacy consultation service as it might be more convenient for you. Thank you for your understanding.
We aim to respond to all queries the same day. Some more routine queries may take us a few days to get back to you. Shotfield Medical Practice working days are Monday to Friday 8am to 6.30pm.
Please wear a mask if presenting with any respiratory symptoms, such as coughing, sneezing, or a sore throat, to help reduce the spread of illness.
Wearing a mask helps protects both yourself and others.
