If you need to access some information from your medical record you may first consider registering for enhanced online access. You can do this via our website and then you need to complete the consent form and drop it into reception with one piece of recent photo ID.
Alternatively if this doesn't serve your needs or you need access to more detailed information you must put that request in writing to the Practice Manager. Please detail exactly what you need to access (ie all or part of your records with dates) and this information will then be reviewed, discussed with a GP and you will be notified of the relevant fee which will depend on the type of access and the number of items required. If you are agreeable to this fee the items will be prepared and made ready for you to collect at reception where a form will be completed by you and the fee paid (cash or cheque only). You should allow 7-10 working days for this entire process to be completed.
