Employed/Paid carers are entitled to the vaccine. Anyone who is employed in social care can now go to NHS - Book your vaccine and book their own appointments, they need to enter their details, selecting they are employed in social care when given the option.
GP Telephone Appointments are available to Patients.
The GP will make a face to face appointment with the patient after telephone triage if they feel it is necessary.

You will be asked if you or anyone in their household has cough, fever, shortness of breath or altered taste /smell. If so they should stay at home and isolate, do not come to the surgery. They can have a telephone review or 111 as needed.
In line with new guidelines from the government ALL patients should now wear a face covering / mask over their nose and mouth when in the building at all times
Please do not arrive early as you will only be let into the building 5 minutes before their appointment (unless very frail etc).
Only patient’s will be let into the building and no relatives please (unless disabled, learning difficulties or under 18 then 1 relative/carer may come into the building).
Please do not delay getting health care. If you need urgent medical help and it is not an emergency, contact your GP or NHS111 online or telephone NHS111 first. During the COVID19 pandemic it’s important that A&E is only used by those with serious or life threatening problems. There are many ways to get the right care at the right time via www.nhs.uk, your pharmacist and your GP.
Your GP offers online, telephone and video consultations. If you are invited in for a face to face appointment, infection control measures are in place to keep patients and staff safe. YOU WILL NEED TO WEAR A FACE MASK.
For those getting ECGs
Due to infection control, and to maintain social distancing in our waiting room, all patients referred for open access ECG need to ring and book an appointment prior to attendance.
Once patients have the request form, please call the department on 0208 296 2575 to arrange an appointment prior to attendance.
Any patients arriving in the department without prior arrangement will be turned away. The 'walk in' aspect of the ECG service is now suspended until further notice.
The Department are currently only seeing urgent patients, and hope to begin a routine service again shortly.
The ECG service at Old Court House will resume its original operating hours next starting on Monday 29/6/20 i.e. Monday and Wednesday 8.15 - 12 midday and 3 – 5.30pm and Thursday 8.15 – 12 midday.
Novocor event monitoring will remain by appointment after referral into service, as before

PLEASE DON’T BE OFFENDED, BUT WE ARE ADOPTING SOCIAL DISTANCE RECOMMENDATIONS AND THEREFORE YOU SHOULD NOT BE COMING TO THE PRACTICE. YOU SHOULD ONLY CALL THE PRACTICE. WE WILL NOT ACCEPT ANY PATIENTS IN THE BUILDING UNLESS THEY HAVE BEEN ASKED TO COME IN BY GP.

Information for patients requesting letters for employers – If you require a sick note / isolation note as a result of suspected coronavirus infection or contact, please go to 111.nhs.uk/isolation-note where you can get a certificate.
Please do not ask us for letters evidencing your health conditions during the coronavirus outbreak. You should speak to your employer and ask them to follow the guidance, and they should risk assess you based on any conditions that you have.
Community Cardiology Services are no longer provided at Jubilee Health Centre, but at The Old Court House Surgery, Throwley Way, Sutton, SM1 4AF.
